Figure 2. In this figure each circle represents a provider. The size of the circle represent the number of patients in an episode of care. Each color represents a specific Healthplan. The area delineated in the red box identifies those providers who have lower relative cost and better outcomes. The larger the effect size number the greater the effect (providers to the right are better). The lower the standardized cost number the less expensive, (Up is good).

It all started when…

Healthcare reform enacted by Congress was implemented to improve patient experience, improve population health and to better manage the cost of health care.

Put simply, Private Practice Cloud, LLC has created a business and analytic platform (Platform) that provides network-enabled services for private practice mental health professionals and point-of-care mobile Apps to drive clinical and financial results to an independent practice association or network of medical and mental health professionals.

Private Practice Cloud offers participating members an array of modules for a suite of services to manage behavioral health records, patient engagement, care coordination and population health.  Participating providers can subscribe to the modules they find essential to operate their practice.  They can change their subscription to include any of a number of modules required to begin contracting.

Inherent in the platform design are process and control mechanisms for contracting and administration that have been vetted by clinicians and attorneys to assure that network and individual members remain within explicit clinical and legal “safe-harbor” guidelines.   In effect, the Platform has policy that manages the risk of violating Antitrust Law and professional licensing regulations.

The Platform Applications (App) are designed and the current implementation is a comprehensive end-to-end application consisting of a

  1. qualified adminstrative process and

  2. models, workflows, data processing, and administrative controls and procedures.

Our Apps are created using best practices in software engineering with world class security to protect provider practice information (PPI) and protected health information (PHI).  The platform is designed to gather and process clinical practice data and create information with use of a world class analytic and reporting system that can support contract compliance, quality assurance and quality improvement programs.  The information gathered by the system can be used for purposes of local and national group contracting.  The platform App can be configured for use as a Software as a Service (SaaS) turnkey model.

The Platform includes

  1. a business plan,

  2. administrative functions,

  3. a value-base contracting strategy and capability,

  4. administrative personnel requirements,

  5. subscription and member management functionality,

  6. separation of concerns and functions into manageable elements,

  7. an App design using best practices in user interface (UI) and user experience (UX),

  8. back-end software architecture and code that supports calculations and data base operations without stall, and

  9. a world class visual analytic dashboard. Measurements and data gathering procedures are reliable, valid and useful; they have value to patients, physicians, therapists, Healthplans and researchers.

Our App has an optional chart note system that can serve multiple purposes. The chart notes system was co-created with payers and providers to assure value to patients, providers and physicians.  There is an export/import protocol that can gather data from other sources including compatible EHRs and Payer information systems.  These protocols are designed to ensure data integrity, privacy and security.  HIPAA assurance is an ongoing platform process; not a static state.  The platform has third party oversight with recognition of industry standard protocols to periodically review, test and document results.

The current state of the Platform’s development is documented in visual diagrams depicting functional modules and functional relationships between components.
